Referrals

HERE TO HELP

We understand that choosing the right home for a child or young person is one of the most important decisions you can make.

Our home is a safe, caring and supportive place for children and young people who may be going through a difficult time. We specialise in supporting children with emotional and behavioural difficulties and those who may find it hard to trust, feel safe, or settle easily.

Here, every child is treated as an individual — with kindness, respect, and understanding.

Who can make a referral?

We accept referrals from:

  • Local authorities and commissioning teams
  • Social workers and placement officers
  • Youth justice and education professionals
  • Emergency and planned placements
Our referral promise
  • Quick response to all enquiries
  • Transparent quoting for placement packages
  • A child-focused matching process
  • Bespoke support plans tailored to individual needs
  • Warm, professional welcome into our home

How to Refer

Step 1

Initial enquiry

Contact us by phone or email to check availability and suitability. We’re happy to discuss the child’s needs and answer any questions.

Step 2

Share information

We'll ask for basic referral documents, such as:

  • Referral form or placement request
  • Risk assessments
  • EHCP or care plans
  • Chronologies and behaviour profiles
Step 3

Needs Assessment

Our team will carefully review the information to ensure we are the right match. We will then provide a clear quotation, based on:

  • The child's care needs and staffing requirements
  • Support for education, health, and emotional wellbeing
  • Any therapeutic or additional services
Step 4

Placement Planning

If the placement is agreed, we'll plan the transition together, including:

  • Welcome visits (where possible)
  • Care planning meetings
  • Introductions to key staff and routines
Step 5

Settling in

We will support the child or young person from day one — with patience, structure, and lots of positive encouragement.

Admission Criteria

Liberi Care will accept referrals that meet the following criteria:

  • Children aged between 10 and 17 years old
  • A child is adolescent and it's decided that they'd benefit from a period of stability and support
  • A placing authority's assessment has determined that a child may benefit from a secure, therapeutic environment
  • A child in need of an alternative placement, including those that are new to Children's Services or have had previous similar accommodation arrangements
  • A child that would benefit from supported accommodation to enable an assessment to be completed

Make a referral

For local authorities, social workers or commissioning teams, we offer a clear, quotative process for all referrals. This includes:

  • Bespoke costings based on individual care needs
  • Support for education, mental health, and behaviour
  • Flexible packages tailored to the child or young person
